Cair Health's AI agents submit claims with Stedi
Cair Health is an agentic operating system for revenue cycle management (RCM) – the administrative work that turns a patient visit into a payment.
Cair's agents handle that cycle from the appointment through to the payment. They check coverage, code visits from clinical notes, scrub claims, work denials, and post payments.
Multi-site clinics, management service organizations (MSOs), billing and RCM companies, and specialty electronic health record (EHR) platforms all use Cair to run their RCM.

Challenge
Cair's platform carries more than 80 integrations. Those integrations let Cair's agents read encounters, notes, and claim data out of each customer's system, and write results back.
The problem is that none of them could submit a claim to a payer.
Cair's agents could retrieve, correct, scrub, and validate a claim, but they couldn't deliver the finished claim to a payer. Customers had to complete that step themselves.
"Our agents could code the claim, create it, and validate it. But then the customer had to submit the claim on their own," says Ishan Balakrishnan, co-founder of Cair Health. "The goal of our agents is automation. We want our agents to own the entire process with no friction."

The handoff also meant Cair had less visibility into the claims lifecycle after submission. Cair couldn't retrieve the claim acknowledgments or Electronic Remittance Advice (ERAs) directly. If a customer wanted Cair's agents to work a denial, they had to feed that information back in.
"Customers had to forward us their remittances," says Ishan. "If a claim was denied, it just showed up again a few weeks later. There wasn't a neat claims lifecycle we could follow."

Cair needed a way to submit claims to any payer a customer might already be billing. Their agents had to be able to do that programmatically, over an API. And Cair needed the payer's responses – acknowledgments and ERAs – over the same integration.
Solution
Cair integrated their agents with Stedi's JSON Claim Submission API. The API gave agents the ability to submit claims programmatically – no customer handoff needed.
The Stedi Payer Network lets Cair submit electronic claims to the payers their customers already bill. Customers can use their existing payer IDs with no mapping required.
Stedi's payer ID aliases automatically map a payer's alternate IDs – including ones the payer has since changed – and route transactions to the right payer.
Cair also uses Stedi's webhooks to get notified of new claim acknowledgments and ERAs. Agents can then use Stedi's API to retrieve the acknowledgment or ERA when working a denied or rejected claim.

Results
Stedi's support helped Cair scale their claim submission functionality quickly.
Cair's claims volume grew by 4.3x in their second month on Stedi. Two months later, Cair's monthly claims volume had increased by 141x.
Cair now submits tens of thousands of claims a month to 199 payers using Stedi's API.

Stedi doesn't just help with submitting claims to the payer. They also help ensure claims aren't rejected.
Stedi uses claim edits to validate claim submissions. Those edits catch errors in claims before they reach the payer. Payers accept 98% of claims that Cair submits through Stedi, due in part to those edits.
"We want to build agents that handle RCM from beginning to end," says Ishan. "To do that, we need a reliable, API-first clearinghouse partner. Stedi gives us everything we need to build the comprehensive agentic billing workflows we want to deliver to our partners."
